A completed Application for Degree (PDF) form must be delivered to the CMES Director of Graduate Studies at least one week before the Office of the Registrar deadlines. In the case of the interim AM in History the student must deliver the application first to CMES, and then to the Department of History before filing the completed form with the Registrar. In unusual circumstances late applications may be accepted for the next two weeks only; there is a $50 fee for late applications.
The Application for Leave of Absence (PDF) is for degree candidates whose time will be devoted primarily to activities other than degree work. It is not advisable for students to take more than one year of leave. In unusual situations where additional leave may be required, the department should contact Rise Shepsle, assistant dean of student affairs, (617-495-1814) to discuss the situation.
The Traveling Scholar Form (PDF) is for students who are away from the Boston area engaged primarily in their degree work. Please note the instructions concerning health insurance coverage and international students.
Students who have been required to withdraw from GSAS may apply for readmission (PDF), for the purpose of receiving the degree, once their thesis has been completed. The application for admission should be accompanied by a letter from your advisor stating that an acceptable thesis has been received. Applications are to be turned in to the Dean's Office, Holyoke Center. Readmission for Degree applications are accepted on a rolling basis, since students may receive a November, March, or June degree.

A Dissertation Committee Form (PDF), signed by the required three committee members, must be filed with the CMES Coordinator of Graduate Studies. Students are required to form a committee no later than two terms after completion of the General Examination.
The Language Certification Form (PDF) certifites proficiency in a written exam, administered by the Committee on Middle Eastern Studies, in the student’s chosen Middle Eastern language.
To be signed by the student’s advisor after approving a dissertation (PDF) prospectus presented at a department workshop or conference. A copy should be filed with the CMES Coordinator of Graduate Studies.
